First Care
(High Risk Infant Follow-up)

First Care provides services to children from birth to 1 year one who have:
- A medical condition which places them a risk for health or developmental problems
- Serious socio-economic conditions impacting their physical health AND who need intensive medical follow-up services
First Care/ HRIFU (High Risk Infant Follow up) provides voluntary in-home or clinic based nursing assessments, interventions, case management, and follow-up for families with infants who need close monitoring of their health and medical status after hospital discharge, who because of a diagnosed health or medical condition , are or may be at increased risk for mortality or morbidity. Services are provided with the consent of the family and in collaboration with and as an extension of the primary care provider as well as specialists and other social service providers involved with the infant/family. Services are provided for birth to one year of age.
